For a cup game you still use this card but you only fill in team names, player names and overall match result e.g 500 - 491. You do not need to put how many wins each player had and who won the doubles. Match comments on how you found the format of the cup competitions would be appreciated.

In order to submit the form you must fill in the image verification at the bottom of the form - this prevents us getting lots of SPAM mail. Some of the images are tricky to see so i suggest before filling in the form you just click refresh image until you find one that is clear.
